  • Information Requested: Last year’s financials, and current year milestones.

  • Preparation: Have your taxes, P&L’s, or your best estimates ready

  • Time Commitment: 20-30 minutes if you have your taxes or P&L’s handy

  • Who fills this out: Any client who wants to continue support

  • When: 10 months after your program enrollment date

  • Format: Google Form from Livelihood NW

  • Information Requested: Interest in further support, feedback to Livelihood NW

  • Preparation: None

  • Time Commitment: 2-10 Minutes


Frequently Asked Questions

  • We collect information from the businesses we serve to demonstrate the impact of our programs and services. Demonstrating our impact is the way we are able to secure funding to be able to provide our programs and services for free & low cost, as well as receive feedback from YOU on what we are doing well, and where we can improve.

    Providing data annually is part of the Engagement Agreement you signed when applying or enrolling in business support with Livelihood NW. The Agreement can be read in full here, and states:

    Financial Reporting: Client will provide business financial reports annually. Surveys will be sent out in April and must be completed before June 1 in order to continue receiving free or sponsor subsidized services. 

  • Private information on any one business or person collected by Livelihood NW is NEVER shared. Instead, we combine data to see trends and report to our funders high level.

    For example, we do not report the number of employees you have, we report that of 500 businesses served, 1,234 jobs were created or retained.

  • If you do not provide the required paperwork, you will not be able to access our programs.

  • It is important that we get your Annual Client Survey by the end of June so that we can begin our data crunching and meet our own deadlines with our funding partners. If you missed the deadline, fill it out as soon as you can, and reach out if you need support.

    Your Support Renewal Request is only required if you are requesting another year of business support.
